Due to Maternity Leave within our friendly, busy Derby City branch, we currently have an exciting opportunity for a confident, outgoing individual with excellent communication skills, to join the Mediline Field Care Co-ordination team on a 12 month fixed term contract (with the possibility to become a permanent position ) .

Annual salary is £21,580, made up of a basic annual salary (£17,500) and additional payment for participation in the Out-of-Hours Emergency On-call Service (Approx £4080)

Skills and experience required
Full clean driving licence and use of own car - Essential
Good keyboarding skills with knowledge of Microsoft Office - Essential
Ability to motivate, guide & coach - Essential
Ability to work to strict deadlines - Essential
Good communication skills, both written and verbal - Essential
Self motivated with the ability to use initiative to solve problems – Essential
Previous experience in the delivery of care – Essential
Minimum NVQ Level 2 or QCF Diploma Level 2 in Health and Social Care – Essential
Experience in supervision of Care Workers/team members - Preferable
Experience of “hands-on” care is an absolute must; unfortunately we are unable to consider candidates without this experience.

REQUIREMENTS
The role of a Field Care Co-ordinator requires the efficient set-up and maintenance of Service User packages of care together with the following duties which include, but are not limited to:
Liaising with multi-disciplinary teams to set-up, develop and maintain care packages for Service Users
Attending Service User reviews, dealing with complaints where necessary
Ensuring that all relevant and up to date paperwork is in situ at the home of Service Users (Risk Assessments, Care-plans, Service User guides)
Proving support to new field based recruits, “shadowing” to ensure suitable levels of competence
Assisting with the creation of rotas for care staff, ensuring that a quality care service is provided at all times.
Conducting appraisals, supervisions, absence management meetings with field based Care Workers
Providing exceptional levels of customer service and care to Service Users, delivering care when required
Operating in a way which promotes dignity and respect to Service Users and other team members
Undertaking Emergency on- call duties on a rota basis (paid in addition to annual salary)
Hours & Pattern of Work 40 hours per week - Core hours 9am to 5pm – Monday to Friday however; flexibility is required either side of these times dependent upon the requirements of the role (e.g. early start, late finish, weekend working). Plus, Emergency on-call cover on a rota basis (Payment in addition to annual salary)
