Make a Referral
If you are looking to refer someone for supported living or care services, our team can guide you through the next steps.
Who Can Refer
Referrals most commonly come from professionals working with individuals who require support to live independently.
Social Workers
Social workers often refer individuals who require supported living or community-based support.
NHS Teams
Healthcare professionals may refer individuals who require additional support in everyday life.
Case Managers
Independent case managers may refer individuals arranging support through private funding.
Local Authorities
Local authority brokerage and commissioning teams regularly refer to Mediline through established procurement platforms.
Direct Payment Holders
Individuals or families managing their own care budget may contact us directly to arrange support.
Before You Refer
To review a referral, our team will usually need
the following information:
- The area where support is needed
- The type of support required
- A brief overview of support needs
- Relevant background information
- Contact details for the referrer
If someone has not yet had a social care assessment, we may advise contacting the local authority first.
What Happens After Submission
Once a referral is received, our team will review the information and respond as quickly as possible.
Initial review
We review the information provided and assess whether Mediline is likely to be a good fit.
Follow-up discussion
Our team may contact the referrer to gather further details and discuss the individual’s needs.
Assessment
Where appropriate, we arrange an assessment to explore support options in more detail.
